Wellness Recovery Action Plan (WRAP), Inc. Request Form

Please complete and submit this form for any WRAP-related requests. Once approved by a WRAP, Inc. team member, you will be notified via email. Only formally submitted requests will be considered.

Examples of requests may be in the following categories:

• Use of WRAP, Inc. logo
• Use of PowerPoint slide decks
• Special WRAP training requests
• Use of website branding
• Use or reproduction of WRAP® materials for purposes other than WRAP seminars
• Facilitator needs (slide decks, materials, etc.)
• Guest presentation or keynote requests
• More information about WRAP® for issues not addressed in FAQ section or on website
• Translated materials
• Permission to represent WRAP® at an event (or request for us to do so)
• Community outreach request
• Scholarship request
• Consideration to become a WRAP, Inc., licensee, Center of Distinction, or subcontractor
